*Parent/guardian to complete steps #1 and #4. Form must completed and returned, along with a brief note outlining the reason for the request, to the league via email at [email protected]. Once received, the request will be forwarded to the District Administrator for approval. A response can take several weeks and, in many instances, may not be answered until just before the start of the regular season.

NOTE: Not all requests are approved. If approved, the waiver grants permission to participate in regular season play only. Little League does not and will not grant out of boundary waiver requests for All Star team play.

Once a response is received from the district administrator, we will inform you as to whether your request was approved or denied.
